Cost of Care

Keep in mind that the cost of Private Case Management may actually save you money by connecting you with useful community resources, including free ones and steering you away from expensive living arrangements where fees may quickly rise without warning. When you have the least, you have the most to lose!

Initial Consultation/Family Meeting $160 (2 hrs.)
Hourly Rate $80.00
Assessment of Activities of Daily Living (ADL’s), Determination of Needs and Care Plan $500.00

When you decide to use Private Case Management;

I WILL:

Request a signed Service Agreement prior to beginning service.
Ensure the confidentiality of client information at all times.
Provide only those services authorized by the client/responsible party.
Provide regular verbal and written reports documenting our findings, recommendations and actions.
Foster the client's autonomy, well being and safety at all times.
Consider the client and customer an integral part of the Care Plan.

I WILL NOT:

Benefit financially from any referral we make to community services.
Provide direct services, such as home health or nursing services.
Provide financial management or fiduciary services.
Act as guardians or conservators.
Accept gifts or tips from clients.
Provide services we are not qualified to provide.
