Private Case Management is personalized service, based on assessments that create a well developed care plan. That plan is developed with you and reflects your choices and preferences for service. The goal is to ensure that you are involved in all aspects of the planning, service arrangements and delivery.

Private Case Management coordinates the planning and service delivery, by consulting with the service providers to ensure that the plan is developed appropriately, clearly contracted and monitored for effectiveness and accountable. Private Case Management monitors the quality of the service you receive.
